The California Healthy Kids Survey / La Encuesta de Niños Saludables de California
Dear Parent or Guardian,
We are requesting your permission for your child to be a part of our school’s California Healthy Kids Survey sponsored by the California Department of Education. This important survey will help provide data to improve the school learning environment, academic achievement, and the health and wellbeing of our youth. It includes questions about drug abuse, safety and violence, risk of depression and suicide, and sexual orientation.
Student participation is voluntary and anonymous. Your child does not have to take the survey. No names or any other identifying information is connected to the answers except for the name of the school.
Student participation requires your permission. We hope you will encourage participation so that your child’s opinion and knowledge can be part of the information gleaned from the survey. If you do not want your child to complete the survey, you must notify the school by returning the attached form.

Le pedimos su permiso para que su hijo forme parte de la encuesta California Healthy Kids Survey que lleva a cabo nuestra escuela y que es patrocinada por el Departamento de Educación de California (California Department of Education). Esta es una encuesta importante que ayudará a proveer información para mejorar el entorno de aprendizaje de la escuela, el logro académico y la salud y bienestar de nuestros jóvenes. Incluye preguntas acerca del abuso de drogas, la seguridad y la violencia, el riesgo de depresión y suicidio, y la orientación sexual.
La participación de los estudiantes es voluntaria y anónima. Su hijo no tiene que contestar. No se relacionará el nombre u otro tipo de información que identifique a su hijo con las respuestas, salvo el nombre de la escuela.
La participación del estudiante requiere su permiso por escrito. Esperamos que anime a su hijo a participar para que su opinión y conocimientos formen parte de la información recopilada de esta encuesta. Si no desea que su hijo conteste la encuesta, debe notificar a la escuela devolviendo el formulario adjunto.
